I'm long gone to Windows Phone after Galaxy S and Galaxy Nexus but I still keep both for development and testing purposes.
I have updated Galaxy S to some popular, quite stable ICS ROM at some time out of curiosity and it was quite a good experience (some things improved, some worse in terms of experience and performance - that's a pretty old phone already so I understand) but I downgraded it back to Samsung's stock TouchWiz ROM to have testing environment similiar to millions of S1 and S2 owners.

As those ROMs improve regularly, I would recommend it to anybody to install custom ROM on old, popular phone. 4.0+ is worth occasional bug.

Because Google's phone, i.e. Nexus, is updated immediately and the rest of updates is up to manufacturers and carriers which add something more on top of Android (like HTC Sense or Samsung's Touch Wiz interfaces), those things need to be updated first to match new system.

In the end - I don't know why anyone would still insist on waiting for those updates from manufacturer as rooting and flashing Android phone with custom ROM is one click process. Maybe there are issues with carriers in US, I don't know, I'm from Europe and I have Gingerbread 2.3.4 on Samsung Galaxy S and video-chat on Skype enabled so I'm just happy.

I would say blame all of them and the process that was created. I'm sure if Google standardized on a release schedule that would help. Then if the carriers/manufacturers would then themselves commit to a schedule based on Google's we would see time updates. I have to imagine they all deserve some fo the blame.
